Please stay up-to-date on reporting requirements for your projects under
the Great Lakes Basin Program for Soil Erosion and Sediment Control. You may submit online Progress and Final reports below.
NOTE: Please make sure to: 1) first prepare and save your reports offline in
a Word (or other) document, such as our Report Template; 2) cut and paste your pre-prepared reports into
the online forms; and 3) print and submit the form as directed. Attempts to
"save" your filled-out online form before hitting the "submit" button can
result in the form resetting and losing your data.

Progress Report
(You may prepare and save your data in advance using our Report Template.) This is a one-step process:
When filling out this form, make sure to click "print" to print page 1/the budgetary cover sheet before proceeding. Sign and mail in this
page.
When your report is complete, click on "submit" at the end. A web page will confirm
whether or not submission was successful.
Final Report
Step 1: (You may save your data in advance using our Report Template.) Complete the pdf form. Make sure to print and sign page 1 before submitting, as instructed. When your report
is complete, click on "submit" at
the end. A web page will confirm whether or not submission was successful and automatically direct you to Step 2.
Step 2: You will then be able to enter in any BMP information (if applicable). You will have the opportunity to enter
in as many BMP's as necessary. You may select the option of not having any (or any more) to enter; this will automatically
take you to Step 3.
Step 3: Submit any peripheral files, including photo images and publication files (please make sure to enter in a caption/title
for each). You will have the opportunity to enter in as many as you need. When you are finished, select "continue" to finalize the submission
of your report. A web page will confirm whether or not submission was successful.
